Description

Bruce House, Wellgate, Arbroath, covering the South East area of Angus (2 positions).

1 position is permanent and 1 position is temporary for up to 1 year from date of appointment;

County Buildings, Market Street, Forfar, covering the North West area of Angus (2 positions - permanent);

Kinloch Care Centre, Carnoustie, covering the South West area of Angus (1 position - permanent);

or

Panmure Street, Brechin, covering the North East area of Angus (2 positions - permanent).

You will work 24 hours per week.

Hours are worked on a shift based system covering 24 hours.

This will include regular nightshift. You may also be required to cover other areas of Angus whilst on shift.

You will be asked to indicate on your application form which position/s you are applying for.

Internal and external applications on a secondment basis will be considered. You must have the approval of your line manager / organisation.

Duties

  • Provide short term support and responsive social, personal and enablement care for individuals in their own homes.
  • Ensure duties are carried out in accordance with departmental policies, standards, operational instructions and professional guidelines.
  • Contribute to the assessment of individuals’ abilities and needs and liaise with care management teams to ensure long term outcomes are achieved.
  • Work in partnership with other services and agencies to prevent unnecessary admissions to hospital and to support individuals on discharge from hospital.
  • Work co-operatively with colleagues, agencies and partners to achieve positive outcomes.
  • Contribute to the maintenance of records in line with appropriate guidance.
  • Contribute to the monitoring of care provision through the appropriate use of technology provided (Call Confirm Live - CM).
  • Promote equality for all individuals by ensuring that their individuality, confidentiality, privacy, choice and rights are respected at all times.
  • Contribute to the health, safety and security of people who use the service.
  • Provide short term support as required to enable carers to continue in the caring role.
  • Participate in reviews, and contribute to assessments, ensuring the needs of individuals and their carers are identified.
  • Support people who use the service and their carers at times of crisis.
  • Participate in team meetings and contribute to effective team work and the flexible    development of the service.
  • To comply with all data protection law in the processing of personal, and special categories of personal data, in line with the council’s full privacy statement and service privacy notices.
  • This list is not exhaustive and the jobholder will be required to undertake other duties as required.

Essential criteria

  • Commitment to undertake appropriate training
  • SVQ Level 2 in Health and Social Care or  willingness to achieve this in an agreed timescale                       
  • Willingness to undertake mandatory information governance training
  • Experience of working with vulnerable adults, within a community care setting and understanding the needs of older people and their carers
  • Experience of carrying out personal care to a high    standard
  • Understanding the advantages of an enablement model of care and how promoting independence benefits individuals
  • Knowledge and understanding of social work values
  • Knowledge and understanding of the needs of carers
  • Good oral communication skills
  • Good written communication skills

  • Ability to obtain and maintain SSSC registration within 6 months
  • Ability to work a 24hr shift pattern
